Why "Busy" Doesn't Mean Profitable in a Chiropractic Practice

A packed schedule feels like success, and often it is. But a full waiting room and a healthy bottom line aren't automatically the same thing, and a lot of practice owners avoid their P&L because it feels like something built for accountants rather than something that actually tells them the truth about how the practice is doing.

Start with revenue, but don't stop there. Revenue tells you how much came in. It says nothing about what it cost to generate that revenue. Subtract direct costs, meaning associate compensation tied to production, supplies, and any direct patient care costs, and you get gross profit, which tells you how much each visit actually contributed before overhead.

Gross margin, meaning gross profit as a percentage of revenue, is one of the more useful numbers to track month over month. If that percentage is sliding even while visit volume climbs, it usually means either fee schedules haven't kept pace with rising costs, or the payer mix has shifted toward plans with less favorable reimbursement.

Below gross profit sits overhead: rent, front desk staff, insurance, marketing, and everything else that keeps the practice running regardless of how many patients came in this month. What's left after all of that is net income, the actual bottom line, and it's the number that determines whether growth in visits is translating into growth in what you're actually taking home.

The mistake we see most often is owners watching the schedule fill up and assuming that automatically means the practice is doing better, without checking whether gross margin and net income are actually moving the same direction as visit volume. Visits can grow while profit stays flat or even shrinks, particularly if payer mix or associate compensation structure isn't being watched closely.

Reading your P&L doesn't require an accounting background. It requires a P&L that's accurate and current, and a few minutes a month spent actually looking at it with these numbers in mind.
