Insurance Reimbursement Lag: Managing Cash Flow When Payments Take 60 Days

One of the biggest differences between running a chiropractic practice and running most other small businesses is the delay between doing the work and getting paid for it. A patient visit happens today. The insurance payment for that visit might not land for 30, 45, or sometimes 60 days, and that gap creates a cash flow challenge that's specific to insurance-based practices.

This lag means your P&L and your bank account can tell very different stories in any given month. You might show strong revenue for August based on visits performed, while the actual cash from a meaningful chunk of those visits doesn't show up until October. Managing this well starts with knowing your practice's real average reimbursement timeline, not a general industry estimate. Different payers move at different speeds, and if a meaningful share of your patient base is with a slower-paying insurer, that specific timeline needs to factor into how you plan cash flow, not an average that doesn't reflect your actual payer mix.

A cash flow forecast that accounts for this lag, projecting when billed revenue is likely to actually convert to cash based on your real payer timelines, is far more useful than a standard forecast built off billing dates alone. This is where a lot of generic bookkeeping falls short for a practice, because it doesn't build in the insurance-specific timing that actually drives your cash position.

It's also worth tracking claims that are aging past your typical reimbursement window, since a claim sitting unpaid at 90 days often signals a problem, a denial that needs appealing, missing information, or a payer issue, that's costing you real revenue the longer it goes unaddressed.
