Every practice deals with no-shows and last-minute cancellations. Most owners treat them as an annoying but unavoidable part of running a schedule and move on without ever calculating what they're actually costing the practice over the course of a year.
A no-show isn't free just because you're not providing care that day. The time slot is gone, whether or not you're able to fill it with another patient on short notice, and the fixed costs of running the practice for that hour, staff time, rent, overhead, keep accruing regardless. When no-show and cancellation rates run high for particular days, particular providers, or particular patient segments, that's a real drag on revenue that doesn't show up clearly unless you're specifically tracking it.
Tracking no-show rate by day of week can reveal scheduling patterns worth addressing, maybe Monday mornings or the slot right after lunch consistently underperform. Tracking it by provider can surface whether certain scheduling approaches or patient communication habits correlate with better attendance. Tracking the total revenue impact of no-shows against total potential revenue gives you a real number for how much of the practice's capacity is quietly going unused.
Once you know your actual no-show rate and its cost, decisions about policy become easier to evaluate. A cancellation fee, a deposit for new patients, or an automated reminder system all have a real cost or effort to implement, and knowing the actual dollar impact of no-shows helps you decide whether a given fix is worth pursuing rather than guessing at whether it matters.
This isn't about treating every no-show as a crisis. Some amount is a normal part of running a practice. It's about knowing your actual rate and cost, so you can tell whether it's within a normal range or eating more into capacity and revenue than it should be.
